Gift Voucher Terms & Conditions –

(By purchasing or redeeming a Gift Voucher you accept these Terms & Conditions)

• Please quote your gift voucher number when making a booking. For online bookings please quote the gift voucher number in the notes section
• A valid gift voucher must be presented at the time of your appointment
• Gift vouchers are valid for 3 years from purchase date (Expiry date noted on voucher)
• Gift vouchers are not refundable or redeemable for cash
• Private Health Rebates cannot be claimed on services provided, when using a voucher
• Vouchers can be used for any service listed, for the time period noted on the voucher
• Vouchers can be redeemed for a service upgrade on a longer treatment by paying the difference in value of the treatments
• The voucher must be used by the recipient named on voucher
• The voucher can be transferred to another person only with the express permission from the original purchaser of the voucher, or the named recipient of the voucher
